A tool to help in FMEA process and to not forget to secure any potential failures...
Start to list the technical functions with their different failure modes and set the main functions of your product/System and its different life situations.
The tool will create all the rows to analyse, by combinating all possible failure modes vs. main functions vs. life situations. !

The sheet FMEA is divided in 3 different functional areas :

Area of the sheet where to set the result of the functional analysis, listing in tables the different life situations, functions and technical functions.
#2 The affectation Matrices area

Area where are defined in matrices the links between the life situations and the functions , and the links between the functions and the technical functions . Those to limit FMEA to only relevant cases.

Area where FMEA is made. checking effects of the different failure modes in the different life situations and assessing the risks to finally define appropriates actions or design controls to reduce the risks to an acceptable level

Add Life situations or functions or technical functions by double click on corresponding buttons in the selection menu area (area #1)

Fill in right hidden columns, closed to the "Add Tech.Function" button, the different failure modes of each technical functions. The failures modes can come from sub-FMEA performed for each technical function.
Create the items in the FMEA table

Double click on "Refresh Table" to create FMEA table
Macro will create one row per possible Life sitation, Function and technical function considering the inputs from affectation matrices and functional analysis
>> Double click on "Refresh table" is required after each adding of items in the functional analysis area>> Double click on "Refresh table" is not required after a midification in affection matrices area

Activate/inhibit the top screen function by clicking on "ON" or "OFF"
This screen helps to know where you are in FMEA table, reminding in a frozen range of cells the active life situation , function and technical function.
Sort & Filter some specific cases in FMEA
Sort & Filter some specific cases in FMEA
Select specific cases in the tables you want to sort and filter in FMEA table by simple click . Cells of the selected items are turned in yellow.
Then click on refresh filter to effectively sort and filter the selected items in the FMEA table.

Depending where the active cell is and the items selected in the functional analysis, a macro updates the display to let appear only relevant areas and columns
Click on black bars to show again the hidden areas

A simple click anywhere on the black bar will make the functional analysis area shown again .
A simple click on black bar above the functional area will make the affectation matrices area shown again.

Failure modes are copy &paste from the “next higher level effect” column of the selected sub-FMEA files

Double click on "+" button to insert and create a new row to set a new action to reduce a risk

Monitor the progress of the FMEA
Work on the FMEA while no red risk is reported in the actual status table of the Dashboard sheet.
First , focus on SO (severity/occurrence) red risks in order to eliminate the maximun of red risks.
Then continue with OD (occurence/detection) red risks, to bring risks of your system to an acceptable level
Sort & filter red risks reported in the dashboard tables by double clicking on one figure of a table
